Most likely culprit is a weak 12V battery. Voltage measurements under load would need to be made to determine if it is good or not.

Connecting directly to the pack voltage at the chademo terminals vs connecting to the terminals inside the Motor Control Unit while ON in READY mode. Check your 12V auxillary battery. Is it the original OEM, or what is the age of it? What is the voltage reading with everything off, and with the key turned to ON, and turned to start and car is READY. If the battery is old and weak then it may be worn out and can't function enough to allow charging...
